The outreach division of Humility HealthCare is actively engaged in community events throughout the year to increase awareness among seniors and their families about the services we provide.
After engaging with potential clients at these events and conducting screenings, our outreach team takes the initiative to carry out individualized home visits. During these visits, detailed explanations are provided regarding the application process for homecare services. These applications are then submitted through various Managed Care Organizations at both county and state levels. Our Outreach Coordinators ensure that comprehensive information is shared about the entire process, including the subsequent stages following the receipt and approval of the client's application, along with a thorough explanation of the programs involved.
